which form DA-1 or R-US 164 for VT on Swissquote

Now I have some amounts of VT on my swissquote account, it is not clear if I shall use the DA-1 like for IBKR or Switch to R-US164 as it seems hinted in the article as it is a swiss broker.
Did you fill a W8-BEN form with Swissquote? Or put another, did they withhold 15% or 30% of your dividends. This is what will matter most I think.
 
looks like they withold 30% total, therefore If I understand well I should fill up both R-US 164 and DA-1 to get back each of the 15% ( I am in Neuchatel, the documents are not merged as it seems in Fribourg)

And what's this all about the DA-1? Do I have to download this from IBKR?

DA-1 is one of the document of your tax declaration ( I attached what it looks like in Neuchatel butmight be different between the Cantons) and in that page,
I put one line per purchase of VT on IBKR,, it is important because it is to get back the money depending of the dividends you had during the year and obviously influenced by when you actually owned the VT.
you need to download the year activity in IBKR so you know what you bought when and use that document as an attachment to the tax declaration :"or instance, I use an annual activity statement report with Interactive Brokers. This contains all operations, all dividends, and the status of my shares at the end of the year. I have never had any issue with that. You can get this from the Reports tab.
